Please supply the following information so that the State Referee Committee (SRC) can work to get a mentor(s) assigned to your event.
When requesting a mentor please be aware of the following.

The Event Sponsoring Organization is responsible to provide accommodations for Friday and Saturday night for each Mentor

The SRC will assign mentor(s) and notify you no later than 2 weeks prior to your event of the assigned and confirmed Mentor(s).
