Please supply the following so that the State Referee Committee can get a clinic set up for your local club.  please consider the following when requesting a clinic:

1. Your preferred clinic date needs to be at least 4 weeks from the date of your request (unless prior arrangements have been made with the SDI).
2. This gives time for the clinic to be created in the online learning center
3. Adequate time for background checks for adults
4. Time for online course work to be completed prior to the field session
5. The field session of the class is 5 hours, so plan your requested start time accordingly (generally clinics are conducted on a Saturday or Sunday and start between 9 and 10 am).
6. The SDI will make sure that your clinic's field session is covered by a licensed instructor, and will provide a second instructor if the class size requires.
7. Please be sure that your clinic's local contact's information is correct and up to date, so that the SDI can more easily communicate with them regarding questions about the course.
